• Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
• Develop and implement individualized care plans to meet the unique needs of each patient.
• Collaborate with healthcare providers to ensure seamless care coordination and communication.
• Document patient progress and care delivered in a timely and accurate manner.
• Participate in ongoing education and training to enhance clinical skills and stay up-to-date on best practices.Requirements:
  • Current, unencumbered RN license in the state of practice.
  • 1+ year of experience in home health or a related field.
  • Ability to travel to patient homes and work in a fast-paced environment.
  • Excellent communication and interpersonal skills.
